Required Routing Checklists

***INDUSTRY SPONSORED RESEARCH NOTICE: The Office of Sponsored Programs is no longer accepting paper routing package submissions for industry sponsored (or otherwise funded) Clinical Trial or Clinical Research Agreements. Going forward all complete routing packages must be submitted through InfoEd. However, please note that the requirement for electronic routing through InfoEd does not apply at this time to Confidentiality Agreements (CDAs) or Material Transfer Agreements (MTAs), or to contract amendments.

For any submission, the Office of Sponsored Programs requires a complete routing package before the agreement can be assigned to the appropriate Contract Administrator for review or negotiation.  For questions related to required routing please contact the Office of Sponsored Programs at 336-716-4548 or contact the Contract Officer who is assigned to your department.

Please note, for all contracts with funding, if a preliminary internal budget is provided at the time of submission, a finalized budget documenting the budget approved by the funding source and the Clinical Trials Office will be required by the Office of Sponsored Programs prior to completion of contract for execution.

Basic Research, Clinical Trial or Services Agreements

The following items are required to be included in the InfoEd submission:

  1. Completed Proposal and Certification Form
  2. Completed, signed Research Agreement Questionnaire (for industry funded projects)
  3. Copy of protocol/statement of work (draft is acceptable if not yet finalized)
  4. Internal budget (preliminary is acceptable if not fully negotiated*)
  5. Proposed agreement, if received (electronic preferred)

Fellowship/Travel/Educational/Conference/non-CME Grants

The following items are required to be included in the InfoEd submission:

  1. Completed Proposal and Certification Form
  2. Internal budget (preliminary is acceptable if not fully negotiated)
  3. Copy of proposed agreement if available

Subcontracts

The following items are required to be included in the InfoEd submission:

  1. Completed Proposal and Certification Form
  2. Copy of protocol/statement of work (draft is acceptable if not yet finalized)
  3. Internal budget (preliminary is acceptable if not fully negotiated)
  4. Proposed agreement, if received (electronic preferred)

If incoming subcontract with industry funding/support, also require:

  1. Completed, signed Research Agreement Questionnaire

Cooperative Group Agreements

The following items are required to be included in the InfoEd submission:

  1. Completed Proposal and Certification Form
  2. Completed, signed Research Agreement Questionnaire
  3. Copy of protocol (draft is acceptable if not yet finalized)
  4. Internal budget (preliminary is acceptable if not fully negotiated)
  5. Proposed agreement or funding sheet, if received

Material Transfer Agreements

A request to put a MTA in place can be initiated via an email to your contract officer. The email should include the following information:

Incoming
  1. Incoming Material Transfer Agreement Questionnaire
  2. Proposed agreement, if received (electronic preferred)
Outgoing
  1. Outgoing Material Transfer Agreement Questionnaire 

Confidentiality Agreements

A request to put a CDA in place can be initiated via an email to your contract officer. The email should include the following information

  1. Proposed agreement, if received (electronic preferred)
  2. Contact name and email address for the outside party
  3. Information relating to the purpose of the CDA

Note: No routing form required. Recommend submission of Confidentiality Agreement Questionnaire.
